We are open from 7:30 a.m. to 5 p.m., Monday through Friday.
What information will I need to make an appointment?
A Tyler CVC scheduler will need the following patient information to schedule an appointment:
- Full name
- Social Security Number
- Date of birth
- Insurance information
- Telephone number
- Description of your symptoms
- The name of the referring doctor, if applicable
- A referral number, if applicable
- If your insurance requires a pre-certification to cover your appointment with Tyler CVC, you’ll need to secure that as well.
What information should I bring to my appointment?
To make your appointment as efficient as possible, here’s what you should know and/or bring with you:
- Drivers license / photo identification
- Insurance information cards
- A copy of your medical records from the physician who referred you – especially any information related to past cardiac treatments
